SPECIALTY SERVICES

Your physician must determine your need for home medical care.

You must need at least one of the following services:

Physical therapy

Occupational therapy

Speech/language therapy

Skilled nursing

You must meet all three of the following home-bound status requirements to be considered for care:

              Normally unable to leave home unassisted

      Leaving home is a tedious effort

           If you leave your home, it can only be for a short period of time and infrequently

We offer a wide variety of home health services

Disease management

Psychiatric nursing

Skilled intervention

Therapy   &  special Services
